Yes. All prescription and non-prescription drugs deemed necessary by the PACE interdisciplinary care team are paid for by the PACE program.
Yes. If a person meets the income and assets limits to qualify for Medicaid, the program pays for a portion of the monthly PACE premium. Medicare pays for the rest. If a person does not qualify for Medicaid, he or she is responsible for the portion of the monthly premium Medicaid would pay. PACE staff will work with the state Medicaid office to determine a person's Medicaid eligibility.
PACE programs provide transportation to the day health center. Transportation is a key part of the PACE benefit.
Transportation is not only provided between the home and the day health center, but also to appointments with specialists and other activities. We encourage families to remain active in the care of their loved one. Therefore we do invite families to share in the provision of transportation.
Participants can attend daily if necessary. Day Center attendance is based on individual needs.
The goal of PACE is to keep participants out of a nursing home as long as possible. If at some point it is in the best interest of the participant to receive care in a nursing home, the participant will be moved into the nursing home and PACE will pay for the care and the supervision of the interdisciplinary team will continue.
Absolutely! We are licensed to cover the whole state except for residents living on Prudence Island and Block Island.
Yes and we sincerely appreciate donations to help cover expenses associated with running the program. People can donate any amount at any time. Families may choose to designate PACE for donations when their loved one, our client dies. If someone wishes to make a donation, they can contact the administration office.
There are many ways to be involved! Enroll in the Program, donate to the program, volunteer at the Program, visit a homebound client, work at PACE! We welcome your support and involvement.
Yes- if a participant moves out of the service area or simply wishes to end their enrollment, they must notify their case manager. Disenrollments occur at the end of the month so the client can be re-enrolled in Medicare and Medicaid programs. In addition, PACE will arrange alternative community services for the participant. While we will do all we can to support a participant the program relies on the participant and family to work together to insure the safety of the participant in the community.
